Position Purpose

The Therapeutic Case Manager supports the Dean of Students Office in safeguarding and advancing students’ welfare, and success. In particular, the Case Manager provides administrative support, program accessibility and case-management assistance for students in Counseling partnering with Health Services and Student Concerns.

Under the supervision of the Director of Counseling Services, the Case Manager will assist in developing, maintaining, and monitoring a clinical case management system aimed at providing continuity of care and follow up for Belmont students.

Job Function

Provide triage and intake evaluations, determining appropriate level of urgency and referral. Assists students with providing rapid triage, close clinical monitoring, evaluating the student’s developmental and psychosocial needs, to effectively and efficiently determine the student’s best fit for available resources, and will ensure linkage/connection to appropriate treatment.

Job Function

Oversee case management/support coordination process with appropriate on and off campus entities, including collaboration with Student Conduct Officer, Title IX Coordinator, Campus Security, and Health Services.

Job Function

Provides day and after hours on-call crisis services as an active team member, consultation to the Belmont community, and relevant outreach to Belmont and/or community partners.

Job Function

Actively assists students with navigating insurance, seeking referrals to community resources, and gaining access to care (including coordination of hospitalization and discharge planning).

Job Function

Assists in the development and maintenance of a database of referral resources and community agencies for the benefit of the students.

Job Function

Assist college faculty and staff with non-crisis student related issues when behaviors present in the classroom, disrupt the classroom, or indicate a student with psychological issues.

Job Function

Assist in development of educational programs, including group therapies for the campus community and pertinent convocations.

Job Function

Develop and prepare incident reports, monthly departmental reports, and statistics related to case management functions.

Applicant must possess the ability to collect, organize, and assess data in a timely and confidential manner using database programs, spreadsheets, electronic communications (email, phone, and web), and direct employee/student/parent interactions.

Experience and comfort in using Microsoft applications (Word, Excel, Access) and the ability to utilize institutional databases to gather necessary information.

Experience working in a fast-paced environment and the ability to respond to high-pressure situations.

Ability to thrive in an environment of hard work.

Ability to work effectively with a wide range of constituencies in a diverse community.

Experience

3 years of professional experience, with demonstrated skills in individual consultation, outreach, and crisis intervention.

Experience in crisis evaluation and safety planning.

Education

Masters degree preferred in mental health counseling or social work. TN licensure.

Physical Requirements and Working Conditions

Sedentary work. Occasionally requires standing, lifting, pulling, pushing, carrying and climbing, reaching both high and low. Frequently requires walking and balancing. Constantly requires sitting, repetitious finger and wrist movement, speaking clearly, hearing conversations, hearing (with acuity), seeing near and far with depth perception and color vision.
